Welcome! My staff and I are fully committed to providing you with the best possible vision care. We are pleased to discuss professional fees with you at any time. Our goal is to make sure you have a clear understanding of our Financial Policy.

REGARDING INSURANCE:

Current insurance cards and co-payments are due at the time of your visit. Health insurance is a contractual relationship between you and your insurance company. Thus, the ultimate responsibility of payment for medical services and materials remains yours. We file insurance claims as a courtesy to our patients. However, due to the many variables and types of plans (i.e. deductibles, share-of-costs, contractual agreements, etc.), it is the patient’s responsibility to know and understand the details of their particular plan(s). The patient is ultimately responsible for payment of balances not covered by insurance.

If you have questions in regards to our charges, please call our office. We will be happy to review your charges with you. Glasses and contact lens fees require a 50% deposit at time of purchase, with balance due at time of dispensing of materials. Your fees will be explained at the time of ordering. If no specific arrangements are made, any account 90 days or older will be referred to outside collections.

Because we provide both medical and routine vision care, we accept a number of benefit plans to help cover the cost depending on your individual needs. Here are just some of the plans that we accept:

To the Patients of Mill Valley Optometry

Due to “shelter in place” and in accordance to the California Optometric Association, our office will be closed at least through April 7th. We expect this situation may extend through April. However, we will be able to receive calls and make future vision appointments.
